I understand that by submitting this application I authorize inquiries to be made concerning my suitability as a CASA volunteer. The information requested in this application and information which might otherwise be obtained will only be used for the purpose of determining my suitability as a volunteer. All information will be held in strict confidence.
Criteria used in the selection of volunteers will be such as to insure the individual is able to meet the responsibilities of a CASA volunteer, including but not limited to time commitment of between 5 – 8 hours per month, contact with staff supervisor, at least 21 years of age, complete training required, advocate for the child’s best interest in all matters.. No individual will be rejected because of race, color, religious creed, national origin, sex, age or marital status.
I realize that failure on my part to meet my volunteer commitment can result in my termination from the CASA volunteer program.
I make a commitment to keep all information and knowledge of the Elkhart CASA program and its services confidential.
I give permission CAPS and the Elkhart County CASA Program to do background checks including local and national police and criminal record checks, Bureau of Motor Vehicle Record Checks, sex offender registry checks and a Department of Child Services record search as part of the application process for prospective volunteers. Additionally, I understand that CAPS may conduct the above named checks randomly throughout my tenure as a CASA volunteer and I give them permission to do so. I understand that my submitting this application and/or completing the required training does not guarantee being accepted as a volunteer in the Elkhart County CASA Program. I understand I will NOT be accepted into the Elkhart County CASA program as a volunteer if:
- Convicted as an adult of either a felony or misdemeanor sex offense, a crime affecting family relationships, child abuse or neglect, assault, battery or a crime against public morals or related acts that would pose risks to children or the CASA program’s credibility
- Found to have charges pending for criminal offenses or offenses listed above
- Refuses to sign a release of information for appropriate law enforcement or Department of Child Services checks
Having carefully considered the opportunities and responsibilities involved, I offer my services as a CASA volunteer in the capacity of a Court Appointed Special Advocate.
